Download the referral form below. Print it to bring to your doctor, or share the link directly with their office.

Your doctor completes it
Your doctor completes the form with your details, your condition, and the reason for the referral.

Call us to book
Once the form is complete, call us to arrange your appointment. We will take care of the rest.
Your right to choose
You have the right to choose your treatment provider. A referral to Genesis Physiotherapy means you will receive care focused on maintaining, restoring, and improving your movement.
Insurance and billing
We accept billing from Colina, NIB, Bahama Health, BAF, First Care, and Atlantic Medical. Payment is due when you receive your service, and flexible payment options are available if you need help.
